Dr. Maclin accepts numerous insurance plans. If your insurance requires authorization for an office visit, please contact your referring physician for that authorization prior to calling for an appointment. If you pursue treatment without the authorization, all charges will be your responsibility. Any patient seen that is not within the scope of our contracted insurance companies, will be responsible for payment at the time of service. Any questions you may have regarding your insurance should be directed to our business office prior to any treatment.
Those patients with high deductibles, that are rarely met during the course of a normal year, if not filing charges to your insurance carrier, may be eligible for reduced, cash-for-service pricing.

Uninsured Patients
Uninsured patients will be treated on a cash-for-service basis. Discounted rates for the uninsured are available. Please contact our business office for details.
The responsibility for payment of fees for services rendered is the direct obligation of the patient. Any deductible or co-payment is the patient's responsiblity, even if Medicare.
